How to Register
1. Complete the blue registration form on both sides.
2. Read the Children's Corner Discipline Policy. Sign and detach the slip at the end of the discipline policy.
3. Read and sign the DCFS Standard Acknowledgement.
4. Send back the completed form, the white discipline slip, the DCFS Acknowledgement and a $50.00 registration fee. Please make checks payable to Children's Corner. Make sure that you have circled the sessions you prefer your child to attend on the blue form. If you do not hear from us, you can assume that your child is registered for the sessions you choose.
5. Make an appointment for a well check with your child's physician. We need all immunizations up to date, a TB Test done no more than 6 months before your child begins school, and a lead screening test and proof of chicken pox vaccination. Thy physical exam form must be signed and dated by your physician within one year before your child starts school. Please send this form in by mail or bring it in no later than your child's first day of school.
